We Have Moved Locations

Dr. Ellis will now be seeing patients at the new location as of 7/6/23. The new location is only one block away from the previous location and the address is:

105B W. Main Suite 115 - Puyallup, WA 98371

The new clinic location is upstairs and there is no elevator. There are two entry door, one at the address location on Main and a 2nd door is located on Meridian next to Caskades at 105B N. Meridian. The Meridian door is closer to our office upstairs.

Puyallup Valley Massage has their own new phone number (253) 237-2008 and booking site, Puyallup Valley Massage - Corey Scott LMP. Please direct all massage therapy inquiries and scheduling to these resources.

What Type Of Chiropractic Appointment Do I Book?

New Patient - Chiropractic

This is for patients that have not seen Dr. Ellis before or have not been in to see Dr. Ellis in the past 3 years.

New Episode Of Care - Chiropractic

This is for existing patients who have seen Dr. Ellis within the past 3 years but not in the past 2 months, or have seen Dr. Ellis within the last 2 months but have a new problem area that was not treated in the past few visits, or you are and existing patient that was involved in a new accident or injury incident.

Chiropractic

This is for someone who is under an active care treatment or ongoing maintenance plan. If you have not been in for more than two months, you would need to schedule a New Patient or New Episode Of Care Visit type.

No Surprises Act - Appointment types will now be offered in Insured vs SelfPay options.

SelfPay fees will be posted for bookable options and reflect a Time Of Service Discount ranging from 20-25% from our regular fees and are due in full at the Time Of Service.
